Request for Medical Files

We are happy to provide files* for insurance, employment , or other medical-related reasons. Your files will then be mailed to you or your designee.

Due to confidentiality, we will only accept requests from recognized insurers or yourself. If you wish to obtain copies of your files, you must visit our offices in person and sign a file release form.
